2012 Registration
[2012 REGISTRATION FORM]
Registration is now open for the 2012 season of Haldimand 3 on 3. This year, we are offering three methods of registration -- (1) online (2) in person (3) fax / email . Regardless of the method of registration, each registration must be accompanied by (1) full payment, or (2) a deposit of $50, with the balance due by March 31. For families with three kids (or more) the third child and each child thereafter will be given a discount of $50 per registration.
We will be offering the following divisions for 2012 -- Tyke/Novice (born 2004 or later), Atom (2002, 2003), PeeWee (2000, 2001), Bantam (1998, 1999), Midget (1995, 1996, 1997).
Instructions for each method of registration is as follows:
1. ONLINE -- We are now able to offer online payments via PayPal. PayPal offers you the ability to pay with your PayPal account or if you do not have a PayPal account, you may use your credit card. If you wish to make payment in this manner, please download and complete the registration form below, fax to 905-765-0055 or email to info@haldimand3on3.com. Once this is complete, come back to this page and click on the button below.

2. IN PERSON -- We will be holding an "in person" registration at the Haldimand County Caledonia Centre on Saturday February 25 from 9:00am to 12:00pm. Please complete the registration form (see below) and bring along with your payment(s) on February 25.
3. FAX / EMAIL -- Please print off the registration form below, complete and fax (905-765-0055) or email (info@haldimand3on3.com) when complete. If making a payment by cheque, send you cheque or money order along with a copy of the faxed/emailed registration form to:
